ATTENTION APPLICANTS
Prior to completing an application, we strongly recommend that you read all the instructions.
It is the applicant's responsibility to download the complete application form.
Incomplete applications will not be processed. All applicants must provide requested information and/or documents as indicated.
Your application must be typewritten.
Application filing fees are non-refundable. Prior to submitting your application, please be sure you have met the minimum requirements (Refer to Instructions for completion of application form as well as the Law, Rules and Regulations.) Letters of Reference:
An envelope marked "CONFIDENTIAL TO THE LAND SURVEYOR TO WHOM PRESENTED" shall be provided to the professional land surveyor completing your professional letter of reference. The envelope shall also reflect your name. The professional land surveyor must complete the reference letter, seal it in the envelope provided, affix his/her signature across the seal of the envelope and return it to you to be included with the application.
An envelope marked "CONFIDENTIAL TO THE PERSON TO WHOM PRESENTED" shall be provided to the two individuals completing your personal letters of reference. The envelope shall also reflect your name. The individual providing the personal reference must complete the reference letter, seal it in the envelope provided, affix his/her signature across the seal of the envelope and return it to you to be included with the application.
We do not accept applications by fax or e-mail. Applications must reflect your original signature.
If you have previously made application to take the Fundamentals of Land Surveying Examination in the State of Missouri, do not complete another application form. Call the Board office for instructions.
Applications must be received in the Board office not later than the filing deadline.
It is your responsibility to keep a copy of the application for your files.
